Overview

  • AHCCCS publishes one fee-for-service (FFS) per diem schedule rather than a rate for each home.
  • It has three resident levels of care (Level I-III) and four areas: Urban (Maricopa/Pima/Pinal), Rural, Flagstaff zips and Tucson zips. 10/1/2026 urban rates are $212.27 (Level I), $232.06 (II) and $275.26 (III); rural $205.65-$266.45.
  • The schedule was originally built from three statewide components: primary care by level and area (nursing minutes x cost-report wages), statewide indirect care and statewide capital.
  • But the state plan has applied no inflation factor since 10/1/2015, so increases now come as ad hoc fee-schedule changes, lately a January 1 minimum-wage bump.
  • Almost all NF days are paid by ALTCS managed care contractors at negotiated rates, with the FFS schedule as the benchmark.
  • Quality DAP adds 0.75-2.25%.

What matters in Arizona

  • One statewide FFS schedule: $212.27-$275.26/day urban (10/1/2026) by resident level, not a rate per home.
  • ALTCS managed care plans (Banner, Mercy Care, UnitedHealthcare) pay nearly all nursing home days at negotiated rates; no FFS floor.
  • The FFS schedule rose only about 3% from 10/1/2023 to 10/1/2026; no inflation factor since 2015.
  • A $20.80 per non-Medicare day provider tax ($2.40 for high-Medicaid homes) funds about $95M a year in per-day supplemental payments.
  • Quality DAP adds 0.75%-2.25% for CYE 2027; 112 nursing homes qualified (41 at the full 2.25%).
  • Medicaid pays the full Medicare Part A coinsurance on SNF days 21-100, not the usual lesser-of $0.

Frequently asked

Does Arizona Medicaid set a separate nursing home rate for each facility?

No. AHCCCS publishes one FFS schedule by resident level (I-III) and area. On 10/1/2026 urban rates were $212.27, $232.06 and $275.26. ALTCS plans, which pay nearly all days, negotiate their own rates with each home.

How does Arizona Medicaid pay Medicare SNF coinsurance for dual-eligible residents?

In full. ACOM 201 requires ALTCS contractors to pay 100% of the Part A SNF coinsurance for days 21-100, even when their Medicaid nursing facility rate is lower than Medicare's payment.

Which Arizona nursing homes pay the lower $2.40 provider assessment?

Homes with enough Medicaid days to pass the federal slope test pay $2.40 instead of $20.80 per non-Medicare day: 21 of 95 assessed homes in FFY 2026. CCRCs and homes with 58 or fewer beds are exempt.

How much is paid outside the posted rate in Arizona?

Moderate: about $95M a year in assessment-funded per-day supplemental payments, roughly 13-14% of base NF spending.

Does Arizona require a minimum share of spending on care?

None found.

Cost basis

Uses cost reports

Yes

Cost report

Nursing Care Institution Uniform Accounting Report (UAR), filed with ADHS within 150 days of fiscal year end (AAC R9-11-204), plus the Medicare cost report if applicable. The UAR now mainly feeds the provider assessment (Medicaid, Medicare and total days; net patient revenue).

Base year and rebasing

No regular rebasing. Under 4.19-D II.5, rates were updated each year by inflation factors from 10/1/2002, but 'for rates effective from October 1, 2011 to September 30, 2013, and from October 1, 2015 and thereafter, no inflation factor will be applied.' Since then the schedule has moved only by ad hoc fee-schedule changes: +1.2% on 10/1/2023, then January 1 minimum-wage (Prop 206) increases of about 0.5-0.8% in 2024, 2025 and 2026. The October 2024, 2025 and 2026 updates were flat.

Not recalculated in recent years; moved only by ad hoc increases.
indirect careSingle statewide rate: the Medicaid-day-weighted mean of facility indirect cost per day, excluding outliers beyond 2 SD (2005 method).Supplies, housekeeping, laundry, food, admin.
capitalSingle statewide rate from capital costs at a minimum 85% occupancy, plus historic property taxes and insurance (4.19-D p.8).No fair rental value; does not vary by home.

System

State index

Applied at

Resident-level

How it works

Each member is billed under the revenue code for their own level of care (191 Level I, 192 Level II, 193 Level III), so each resident's day pays that level's rate (FFS manual ch. 22; 4.19-D p.8).

Classification

Three AHCCCS levels of care (I, II, III) set by the ALTCS MCO case manager on the Uniform Assessment Tool (AMPM 1620); the PAS decides ALTCS eligibility only, plus negotiated rates for ventilator-dependent, sub-acute and specialty care. Not RUG or PDPM.

Applies to

The whole per diem: the primary care component differs by level, while indirect care and capital are the same for all levels.

Acuity measured from

Not applicable: there is no facility CMI. Each member's level comes from the MCO case manager's UAT review (at least every 180 days in a nursing home).

Updated

Per member when reassessed; the schedule itself changes on October 1 and January 1.

Move to PDPM

None. Arizona never based NF payment on RUG case mix, so the October 2023 end of CMS RUG support forced no change.

Verified October 11, 2026

Low-acuity residents

None. Level I ($212.27 urban, 10/1/2026) is the lowest paid level; there is no reduced or flat rate for low-acuity residents. Services to a member who needs a lower level of care than they are receiving (per PAS/reassessment) are not covered (FFS manual ch. 22 Limitations), and people who do not meet NF level of care are not ALTCS-eligible. Checked: 4.19-D, FFS manual ch. 22, FY27 rate file.

Verified October 11, 2026

Capital

Included in the statewide price: one statewide capital component (depreciation, leases, interest, property taxes; at least 85% occupancy in the calculation), the same for every home and level (4.19-D). No fair rental value system.

Verified October 11, 2026

Budget adjustment factor

No explicit across-the-board factor, but 4.19-D II.5 suspends the inflation update: none was applied 10/1/2011-9/30/2013 or from 10/1/2015 on. Schedule changes since then are discretionary (e.g. +1.2% on 10/1/2023, January 1 minimum-wage increases, flat Octobers in 2024-2026).

Detail

Limited. (1) IHS and PL 93-638 tribal nursing facilities are paid the federal OMB all-inclusive rate per Medicaid day instead of the schedule (4.19-D I.A). They are also exempt from the assessment (if on a reservation) and from DAP. (2) Arizona State Veterans' Homes are exempt from the assessment and ineligible for NF-SP. Nursing Facility Supplemental Payments (NF-SP, AAC R9-28-703), funded by the NF assessment plus federal match. Each quarter AHCCCS splits the fund between MCO-paid and FFS-paid Medicaid bed days. The MCO share is a 'per bed day enhanced support uniform increase' paid through ALTCS contractors as a 42 CFR 438.6(c) directed payment, 98% passed through within 20 days. The FFS share is 99% paid directly by bed-day ratio. CYE 2027 ALTCS-EPD estimate: about $95.4M including premium tax (preprint submitted; CMS approval pending as of 7/30/2026). ACOM 201 (eff. 10/1/2023) says contractors 'shall pay 100% of the member cost sharing amount for any Medicare Part A SNF days (21 through 100) even if the Contractor has a Medicaid Nursing Facility (NF) rate less than the amount paid by Medicare.' The FFS manual (ch. 22) likewise says AHCCCS pays the full Medicare coinsurance minus other TPL and share of cost. For other services, cost sharing plus Medicare's payment is capped at the contractor's rate (lesser-of). ACOM 201 covers Medicare Advantage members, but whether the 100% SNF exception also covers MA SNF copays was not confirmed.

Sources: each state’s Medicaid nursing facility rules and statutes, its Medicaid state plan (Attachment 4.19-D) and its Medicaid agency’s rate pages, listed under each state with the date researched. Posted rates are not always what a home is paid: case mix, add-ons, supplemental payments and managed care contracts change the amount. BARA Analytics is independent and is not affiliated with any state Medicaid agency or CMS.

Medicaid managed care and commercial rates are not set by the state. For what commercial insurers publish about nursing-home rates, and what stays private, see SNF payer rates from Transparency in Coverage data.
