Overview

  • Nebraska pays each home a cost-based prospective rate from a base-year FA-66 cost report (rebased at least every 5 years), with a separate rate for every PDPM nursing level of care.
  • The rate is: direct nursing cost per weighted day x the resident's PDPM nursing weight (floor 77%, cap 105% of the urban or rural median), plus support services (floor 72%, cap 100% of median), plus fixed cost (cap $27/day before property taxes), plus the provider-tax pass-through, plus a star-rating Quality Measures add-on of $0-$10/day.
  • Residents in the two lowest groups (PA1, PA2) are paid the assisted-living waiver rate instead.
  • In the 7/1/2026 file (178 homes) the median rate is $115.77 for PA1, $253.20 for CA1 and $620.54 for ES3.

What matters in Nebraska

  • Every home has 26 rates, one per PDPM nursing level; each resident is paid by their own assessment, not a home average.
  • Lowest-acuity residents (PA1, PA2) get the assisted-living rate: median $115.77/day versus $253.20 for the next group up.
  • Nursing home care is fee-for-service; Heritage Health managed care plans do not pay long-term nursing home days.
  • Direct nursing is capped at 105% of median and floored at 77%, so most homes get close to their own cost.
  • The provider tax rose from $3.50 to $9.00 per non-Medicare day in 2024; it is passed back per day in every rate.
  • Quality add-on pays $10.00, $6.75 or $3.50 a day for 5, 4 or 3 CMS QM stars; 1-2 stars get nothing.

Frequently asked

Does Nebraska Medicaid pay one nursing home rate per facility or per resident?

Per resident. Each home has a separate rate for each of 25 PDPM nursing groups plus a short-stay default, and each resident's rate follows their own MDS. In the 7/1/2026 file the median runs from $115.77 (PA1) to $620.54 (ES3).

How does Nebraska pay for the lowest-acuity nursing home residents?

Residents in PDPM groups PA1 and PA2 (levels 201-202) are paid the Aged and Disabled Waiver assisted-living rate plus the provider-tax and quality components, about $104-$123/day, less than half the next group's rate.

Do Heritage Health managed care plans pay Nebraska nursing homes?

No. Heritage Health plans (Nebraska Total Care, UnitedHealthcare, Molina) cover physical, behavioral and pharmacy care, but nursing home long-term care and hospice room-and-board are paid fee-for-service by DHHS.

How much is paid outside the posted rate in Nebraska?

Small: only city/county homes and IHS homes can draw cost settlements above the caps; no managed care or directed payments.

What does Nebraska Medicaid pay on Medicare coinsurance days?

Lesser-of: Medicaid pays only the amount by which its rate exceeds Medicare's payment, capped at the coinsurance.

Does Nebraska require a minimum share of spending on care?

None found; no minimum share of the rate must be spent on care or staff.

How is the Nebraska nursing home provider tax charged?

$9.00 per non-Medicare day; veterans homes, homes with 26 or fewer beds and CCRCs are exempt; high-Medicaid homes pay less.

Cost report

Nebraska Form FA-66 'Long Term Care Cost Report' (state form), filed every year for the July 1-June 30 report period. Medicare HIM-15 principles apply, except chapter 15 (change of ownership).

Base year and rebasing

Rates come from a base-year cost report (report period ending June 30). The base year is updated 'no less frequently than every 5 years' (4.19-D p.15, NE 25-0015). The inflation factor is updated every year. Component medians, caps and floors are set from audited full-year reports after desk audit and are not revised later. The current base year was not stated in the posted plan.

Direct NursingBase-year allowable direct nursing cost (FA-66 lines 94-103) / base-year weighted resident days, x inflation factor, x the level-of-care weight. Floor 77%, cap 105% of the Care Classification (urban/rural) median. Waivered homes (24-hour RN waiver) have the median and cap cut 2%.Medians use homes with an average census of 40 or more, excluding waivered and part-year reports. Urban = Douglas, Lancaster, Sarpy and Washington counties.
Support ServicesBase-year allowable support cost (admin, dietary, plant, laundry, housekeeping, social services, medical transportation, respiratory therapy) / total inpatient days, x inflation factor. Floor 72%, cap 100% of the Care Classification median.Median excludes hospital-based, waivered and part-year homes.
Fixed CostHome's own interest, depreciation, amortization, rent/lease and property taxes / inpatient days, capped at $27.00/day (excluding personal property and real estate taxes). An exception can be requested if total fixed costs rise 20%+ over the base year.No inflation. For homes bought on or after 7/1/2020 the asset basis is the lesser of purchase price or the seller's Medicaid net book value (no step-up). Interest on debt above 80% of asset cost is not allowed (except government homes).
Nursing Facility Quality Assessment ComponentAnticipated provider-tax payments for the rate year / total anticipated patient days (including bed-hold and Medicare days), from the four most recent quarters. Not subject to any cap. $0 for exempt homes.Added to every level of care, including the assisted-living levels 201-202.

Inflation

State-set inflation factor, updated every year and applied to the Direct Nursing and Support Services components from the base year. It is set to spend the appropriation: Neb. Rev. Stat. 68-949(4) (LB130, 2024) says the total amount appropriated for NF rates must be used in the rate calculation, including the annual inflation factor, and DHHS must report how it was calculated by August 1. SFY2026 (7/1/2025-6/30/2026): +1.49% (NE 25-0015). For comparison, SFY2019 was -7.17% (2019 baseline). The SFY2027 factor was not found.

System

PDPM

Applied at

Resident-level

How it works

Each home has a separate per diem for each level of care (4.19-D p.13: 'one rate corresponding to each level of care'), and a resident's claim pays the rate for their own PDPM nursing group from their latest MDS. The 7/1/2026 file lists 26 rates per home (levels 201-280).

Classification

PDPM nursing component, 25 groups (levels 201-272) with the CMS nursing CMIs: PA1 0.66 up to ES3 4.06. Plus level 280 'Short-Term Stay' (0.59) for stays under 14 days with no full admission MDS (admissions from 7/1/2023). Default with no MDS is level 280. Levels 201 (PA1) and 202 (PA2) are paid the assisted-living rate (see low_acuity_rates).

Applies to

Direct Nursing component only (cost is divided by weighted days, then multiplied by each level's weight). Support, fixed, provider-tax and quality components are the same for every level.

Acuity measured from

For setting the direct nursing per diem, all residents' MDS days at each level x the weight, scaled to the cost report's total resident days, give 'Weighted Resident Days'. For payment, the resident's own level applies. A level is effective from the first day of the month of the assessment if received by the 10th of that month; audit changes are retroactive.

Updated

Per resident with each MDS assessment (admission, quarterly, annual, significant change); the per-level rates change semiannually (July 1 and January 1).

Move to PDPM

Moved from RUG-based levels (101-180) to PDPM nursing levels (201-280) effective 7/1/2023 (SPA NE 23-0008, approved 11/1/2023), with a 'federally approved patient driven payment-model grouper'.

Verified October 11, 2026

Low-acuity residents

Yes. Residents in the two lowest PDPM nursing groups, PA1 (level 201) and PA2 (level 202), are paid 'the applicable rate in effect for assisted living services under the Home and Community-Based Waiver Services for Aged Persons or Adults or Children with Disabilities adjusted to include the nursing facility quality assessment component and quality measures component' (4.19-D p.2 and p.15a, NE 23-0008, eff. 7/1/2023). The urban/rural single-occupancy assisted-living standard rate is $3,167/month (about $104/day) for SFY2027. In the 7/1/2026 file, levels 201/202 pay $104.12-$123.12 (median $115.77), against a median $253.20 for CA1 (level 240) and $219.17 for the short-stay level 280. Before PDPM the same rule applied to RUG levels 101-104 (2019 baseline). Separately, short stays (under 14 days, no admission MDS) are paid at level 280 (weight 0.59).

Verified October 11, 2026

Capital

Actual cost, capped: fixed cost per day (interest, depreciation, amortization, rent/lease, property taxes) / inpatient days, up to $27.00/day excluding property taxes. No fair rental value. No step-up in basis on purchases from 7/1/2020. Exceptions are possible for 20%+ fixed-cost growth. Special-needs units have a $54/day fixed cost cap (2019 baseline).

Verified October 11, 2026

Budget adjustment factor

No explicit across-the-board cut. The annual inflation factor is the budget lever: it is set to spend the NF appropriation (Neb. Rev. Stat. 68-949(4)). It was -7.17% for SFY2019 and +1.49% for SFY2026. Caps were also tightened from 125%/115% of median (2019) to 105%/100%, with new 77%/72% floors.

Detail

City- and county-owned and operated NFs can receive the federal share of allowable costs above the rates paid for direct nursing, support and fixed cost for all Medicaid residents; homes with a 40%+ Medicaid mix can recover the federal share of costs above the component caps. Settled after audit, within the UPL. IHS/tribal NFs have a similar provision with quarterly interim payments. Government homes are also exempt from the 80%-of-cost loan interest limit. The number of qualifying homes and amounts were not found.

Funding

Only the federal share is paid (the public owner's own spending on costs above the caps acts as the non-federal share; certified public expenditure in effect). No lease or management arrangement rules were found.

Rule

Supplement 1 to 4.19-B (NE 17-0007, eff. 7/1/2017): for state-plan services, if Medicare's payment equals or exceeds the state plan rate, Medicaid pays $0. Otherwise it pays the lesser of (Medicaid rate minus Medicare paid) or the Medicare coinsurance and deductible. Applies to QMBs and other duals for Part A and B. Medicare Advantage cost-sharing treatment for NF days was not found.

Effect

Medicare's SNF rate is well above Nebraska's per-resident Medicaid rate (median $253-$311/day for mid-acuity groups), so homes usually receive $0 of the day 21-100 coinsurance.
